The purpose of Community Stabilization is to support people during a mental health crisis and help prevent:
 

• the need for more intensive treatment

• major setbacks in daily life

• psychiatric hospitalization

• involvement with the legal system

• substance use relapse

• loss of housing

• 24/7 crisis response and support whenever help is needed.

• Personalized treatment plans created around individual goals and challenges.

• Guidance connecting to community resources such as housing, jobs, or benefits.

• Advocacy and support within medical, social service, and mental health systems.

Who is Eligible:Individuals of all ages who are experiencing a mental health crisis (children and adults).
